Frozen Shoulder: Diagnosis, Treatment, and the Role of Physiotherapy

    Frozen Shoulder

It is a degenerative condition of the glenohumeral joint. It is also known as adhesive capsulitis, which may result in severe pain and loss of movement after some time stiffness increases, limits the movement of the shoulder, and pain stays for a few months.

    Cause of Adhesive Capsulitis:

    • Acute Injury
    • Inflammation
    • Diabetes
    • Shoulder Injury
    • Stroke, etc

Freezing Stage: Shoulder pain in movement and slow loss of movement. This stage may last for 3–5 months.

Frozen Stage: Shoulder pain with restriction in movement; frozen stage stays for 6–9 months. During that time, the shoulder became stiffer and made daily activities very difficult.

Thawing Stage: This stage lasts for 6 months to 2 years; at this stage, the patient may lose overhead activities and feel discomfort. Over time, the shoulder slowly improves its motion and returns to the normal stage or close to normal strength and motion.

    Diagnosing the frozen shoulder:

    • X-ray
    • CT-Scan
    • MRI
    • Musculoskeletal Ultrasound

    Shoulder Special Test:

    • Mozion’s maneuver
    • Apley’s scratch test
    • Painful arc test
    • Lift off test
    • Empty can test

Various manual tests may be performed by doctors or physiotherapists to detect the depth and stage of the disease and the required frozen shoulder physiotherapy.

    Treatment of frozen shoulder:

Frozen shoulder is often treated by nonsurgical methods to improve mobility and pain. Various methods could be applied:

    1. Activity modification
    2. Pain medication
    3. Anti-inflammatory medication
    4. Hot or cold therapy
    5. Steroid injection
    6. Physiotherapy to improve shoulder pain

If adhesive capsulitis is not improving by nonsurgical methods, shoulder arthroscopy or shoulder manipulation by anesthesia can improve a frozen shoulder.

    Exercise for frozen shoulder:

    • Towel stretch
    • Cross-body reach
    • Shoulder rotation
    • Armpit stretch
    • Doorway stretch
    • Stand facing a wall.
    • Pendulum stretch
    • Finger walk
    • Inward rotation
    • Outward rotation
